The Terms and Conditions agreement needs to be completed and signed prior to your dog's first stay with Tanner's Bed & Biscuit. Please sign and bring with you when your dog comes for his/her first visit.

The Client Profile Form is used to help assess if Tanner's Bed & Biscuit is an appropriate environment for your dog. For the health and safety of your dog and others staying with us, please answer all questions as accurately as possible. Once completed, this form will be kept on file.

The Boarding Check-In Form contains information that may change with each of your dog's visits (for example, where you may be reached while away), so this form should be completed when you drop your dog off.
